Turn the PDF text layer into a Markdown file.
Headings are guessed from larger text. Columns, tables, and scanned pages are not reconstructed. If copy-paste from the PDF is empty, run OCR PDF first and use that text.
Each page starts with a Markdown heading so you can see where a page broke.
